Extruded lumbar disc herniation (ELDH) may result in root compression due to the complete tearing of aunulus. The clinical picture in tis case is low back pain with radiating to the involved leg. Our aim in this study was to show the effect of conservative treatment in patients with ELDH. This study was performed prospectively on 15 patients, 6 males (%40), 9 females (%60) who had been diagnosed as lumbar disc herniation (LDH) clinically and radiologically with magnetic resonance imaging (MRI). All patients were treated with syntetic ACTH therapy for 15 days and also physical therapy was performed. Clinical parameters such as pain and functional status were recorded before and after treatment and at control approximately after two months. Level of extruded disc herniation was L4-L5 in the most of subjects. In all patients on the post treatment and the control, both pain and func-tional status showed highly significant improvement (p<0,001). %46,6 of the patients were assessed as excellent, %40  were good, %6,6 (1 patient) was fair and %6,6 (1 patient) was poor. As a result, we concluded that conservative treatment in ELDH is effective.
