Os acromiale results from the failure of fusion of the scapular acromial secondary centers of ossification. It is usually an incidental radiological finding but, symptomatic os acromiale has been also reported. It has been associated with rotator cuff disease; impingement syndrome without rotator cuff tearing has been less commonly described. The treatment of os acromiale is controversial. Conservative treatment with rotator cuff strengthening, stretching exercises, anti-inflammatory medications and steroid injections and if it fails surgical intervention is recommended. We reported a patient with shoulder impingement syndrome secondary to an unfused os acromiale. Os acromiale should be considered in the diagnosis of impingement syndrome.
