Rheumatoid arthritis is an autoimmune disease which is progressive, systemic, chronic, characterized by erosive sinovitis, having unknown etiology and involving joints symmetrically. Rheumatoid arthritis often affects peripheral joints but its axial skeleton involvement is rare. While cervical joints are involved in the vertebral column, sacroiliac joint involvement can be seen rarely. In this case report, a rheumatoid arthritis case with bilateral sacroiliac joint involvement is discussed with differential diagnosis together with the relevant literature.
