Back pain caused by spontaneous spinal hemorrhage is rare. The patient was admitted with the complaint of severe back pain that started following a vomiting attack and the pain remained unresponsive to the any analgesics given. Healso had the signs of neck stiffness on physical examination. An MRI was performed in order to make a differential diagnosis and the findings of this investigation were consistent with an acute subarachnoidal hemorrhage in the D10-L3 spinal cord-conus medullaris region. We present the case of this patient who has been consulted with the related specialists and has still been followed up
