Backache during pregnancy is a frequent complaint at least in %50 of all pregnancies. In only 1:10.000 of pregnancies herniated lumbar disc is the cause of the pain. A 26 years old patient presenting at 16 weeks of pregnancy with left sided paresis on the foot is described. Lumbar magnetic resonance imaging revealed L4-L5 extruded disc herniation. She has been operated and postoperative period was uneventful. Patients presenting with radicular pain during their pregnancy can be evaluated with MRI during the second and third trimester safely. Herniated lumbar disc surgery in pregnant women can be done safely in left lateral decubitus position without any risk of developing vena cava inferior syndrome. In pregnant women presenting with progressive neurological deficit and herniated lumbar disc surgery should be planned before symptoms of cauda equina syndrome develops.
