İnmeli Hastalarda Üst Ekstremite ve El Fonksiyonlarının Değerlendirilmesi
The Evaluation of Upper Extremity and Hand Functions in Patients with StrokeThis study was planned to observe the improvement of motor impairment, spasticity and dexterity of stroke inpatients on rehabilitation programs, and to investigate the relationship among the assessed impairments and functional independence level. Forty-three stroke patients who have been hospitalized in Ankara University Medical School, Department of Physical Medicine and Rehabilitation during 1994-1996 were included in the study. Demographic and clinical characteristics of the patients were documented. Motor impairment and spasticity were evaluated by Brunnstrom’s motor assessment and Modified Ashworth scale, respectively. Grip and pinch strength were measured by dynamometer and pinchmeter, dexterity was assessed by Nine Hole Peg Test. For functional independence evaluations self-care subitems of Functional Independence Measure (FIM) were used. Statistically significant improvement of all assessed clinical parameters was observed at the end of the three months. Assessments of grip and pinch strength and dexterity were not sufficient and reliable for the stroke patients with severe motor impairment and spasticity. A high correlation was observed between the motor impairment and functional independence levels. After the rehabilitation programs, patients with stroke may still have severe motor impairment of upper extremity and hand, leading to dependency in daily living activities. In addition to standard rehabilitation programs, special therapeutic interventions for upper extremity and hand should be considered in stroke rehabilitation
