Localized hipertrophic mononeuropathy is a focal neuropathy characterized by concentric proliferation of Schwann cells around the axon. Such lesions are very rare in the absence of generalized hypertrophic neuropathy. Very little is known regarding the etiology and the natural course of this disorder. Our case had progressive focal neurological deficits that localized to tibial portion of sciatic nerve. Serious axonal loss and denervation findings were found in electrophysiologic evaluation suggesting a focal lesion of the tibial nerve. Magnetic resonance imaging from lower lumbar spine to the popliteal region demonstrated marked enlarged sciatic nerve showing abnormal hyperintense signal along thigh segment. The patient was diagnosed as the localized hypertrophic sciatic neuropathy. Pain was decreased by using medical and physiotherapy modalities, but muscle weakness progressed during the follow-up period.
