The incidence of atlantoaxial subluxation in children with Down syndrome has been reported as 10-21% in the literature. Hypoplastic C1 arch, clival odontoid irregularity and the presence of atlantoaxial instability have been termed as "the triple jeopardy". Routine clinical follow up of Down syndrome, however, does not involve this condition which can lead to serious neurological sequels. Considering the case presented, this report aimed to draw attention to the vital importance of atlantoaxial subluxation for children with Down syndrome.
